HUMAN ERRORS

Human errors can be divided into two categories:

  • Latent errors: Latent errors or failures typically stem from decisions made by designers, manufacturers, and senior management who are often distant from the immediate operational system. However, the outcomes of their actions or decisions, which may have been dormant and undetected for a considerable period, can suddenly lead to disastrous results. The consequences of latent errors are not immediate.

Active erros → obvious with immediate consequences
Latent errors → hard to detect without immediate consequences
